Job Details:
- Schedule: 8-hour days, starting from 09/08/2026, with a minimum of 40 hours per week.
- Required Qualifications: Valid state licensure as a Physical Therapist (PT), BCLS certification, 1 year of skilled home health experience, reliable transportation, and experience with Oasis documentation systems.
- Preferred Qualifications: Home health experience preferred.
- Additional Information: Traveler is expected to have their own transportation. The facility utilizes Oasis for charting. The work may require occasional travel within a maximum radius of 50+ miles, with efforts made to group clients to minimize driving time. The position does not require on-call hours or weekend work.
