Job Summary:
Physical Therapist Outpatient (PT OP) in Fort Collins, Colorado. This outpatient therapy position involves evaluating patients, developing and implementing personalized treatment plans, and promoting functional recovery. The role emphasizes patient safety, professional development, and adherence to organizational standards, with a focus on delivering high-quality outpatient care.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active and applicable license for the state of Colorado; previous assignment with the agency or organization highly desired; desire for a 6-month duration assignment highly desired.
• Demonstrates skills in completing comprehensive patient evaluations and developing treatment plans aligned with patient needs and physician directives.
• Maintains accurate, timely medical record documentation, including goal setting, functional assessments, and charge entry, in accordance with state, federal, and organizational policies.
• Participates in student supervision and creates a positive learning environment.
• Pursues ongoing professional development through continuing education and involvement in professional organizations.
• Complies with state practice acts and maintains licensure in good standing.
• Understands and participates in organizational performance improvement initiatives, providing feedback to enhance service quality.
• Adheres to organizational safety and infection control policies, including safe patient handling and workplace hazard awareness.
