Job Details

• Position Description: Provides physical therapy services to patients in a home health setting across eight counties, including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, and Pocahontas. Emphasis on general ortho, weakness, strengthening, and cardiac rehab patients.
• Certifications and Education: Current BLS certification required. Must be licensed to practice in the state of West Virginia.
• Experience: Minimum of 2 years of experience as a physical therapist in inpatient or home health settings.
• Schedule: Day shift, 8:00 AM 4:30 PM, Monday through Friday, with a minimum of 40 hours per week. Self-scheduling is permitted, allowing flexibility in patient assignments within operational guidelines.
• Additional Information: Orientation held at the Gassaway office to familiarize with Epic documentation system tailored to clinician needs. No weekend or holiday work required.
• Submission Requirements: License to submit; willing to wait for license confirmation.
• Notes: Travel may involve covering multiple counties; flexibility is appreciated given coverage areas and mileage considerations.
