Job Summary:
A Physical Therapist specializing in Skilled Nursing Facility (SNF) settings is needed in Haverhill, Massachusetts. This position involves evaluating and rehabilitating patients with physical functioning disorders and wound care, developing individualized plans of care, administering procedures, and monitoring patient progress. The role emphasizes adherence to all regulatory and clinical practice standards to deliver high-quality patient care.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: 1 year of physical therapy experience, Massachusetts state license, experience in long-term care or rehabilitation settings, new graduates with Level II experience are acceptable
• Preferred Qualifications: None specified
• Float: Float rotation may be required
• Additional Information: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Administers procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive device use, and wound care. Assesses patient progress and modifies treatment plans accordingly, documenting all care in the electronic system. Participates in meetings, trainings, and conferences as required. Supervises support staff and ensures compliance with professional practice acts, ethics, and organizational policies.
