Description
Job Summary
Speech Language Pathologist (SLP) specializing in Skilled Nursing Facility (SNF) settings will be responsible for screening, evaluating, and rehabilitating patients with communication, cognitive, or swallowing disorders. The role involves developing personalized care plans based on standardized assessments and evidence-based practices, documenting patient progress, and providing consultation to patients, families, caregivers, and healthcare team members. This position requires active licensure and adherence to professional and regulatory standards.
Job Details
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Active Texas State Licensure, Certification of Clinical Competence (CCC) as a Speech-Language Pathologist, 1 year of Speech-Language Pathology experience, LTC or Rehab experience, and ability to supervise and support CFY or Student Speech-Language Pathologists. New graduates with Level II experience are acceptable.
- Preferred Qualifications: Three years of experience holding active CCS certification.
- Additional Information: Candidates must participate in required meetings, trainings, and conferences. Ability to obtain necessary Durable Medical Equipment (DME) or supplies per regulatory standards. Must adhere to ASHA and State practice acts, ethics, and code of conduct. Documentation of treatment minutes and patient progress must be maintained in electronic records.
