Description
Job Summary:
Occupational Therapist (OT) specializing in Home Health in an outdoor environment. This position involves providing therapeutic services directly in patients homes, assisting them in regaining independence in daily activities such as self-care, leisure, and work. The OT will work under the supervision of the Home Health and Hospice Manager, collaborating with an interdisciplinary team to develop and implement patient-centered treatment plans.
Job Details:
- Schedule: Days shift, Monday through Friday, 8:00 AM to 4:30 PM, with a minimum of 40 hours per week.
- Required Qualifications: Must be a graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Must be eligible to take the National Registration Examination of the American Occupational Therapy Association. Current licensure by the State of Oregon as an Occupational Therapist. Valid driver's license. Current certification in Basic Life Support (BLS). Proof of auto insurance.
- Preferred Qualifications: Registration as an Occupational Therapist with the American Occupational Therapy Association. Evidence of continuing education relevant to physical disability, rehabilitation, mental health, and home health practices. CRASE training required if the travel assignment extends beyond six months.
- Additional Information: The Occupational Therapist will evaluate patients functional status, interpret plans of care, and communicate findings with the interdisciplinary team. Responsibilities include conducting assessments, providing patient and team education, documenting treatment activities accurately, and adapting to emergency situations. The employee supports the organizations mission, vision, values, policies, and procedures, and participates in required education programs related to DNV standards.
