Description

Job Summary:
Outpatient Physical Therapist (PT OP) position located in a healthcare organization. The role involves evaluating patients and developing, implementing and modifying individualized treatment plans aimed at promoting functional recovery and successful return to community activities. The therapist will also serve as an educator for patients, families, and team members, ensuring adherence to safety and professional standards.

Job Details:

  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
  • Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ years of outpatient clinical experience; CPR certification; active license for the state where work will be performed
  • Preferred Qualifications: Successful previous assignment with the organization; experience in outpatient settings
  • Completes comprehensive evaluations and develops appropriate treatment plans aligned with physician orders and insurance requirements
  • Documents medical records clearly, accurately, and promptly, including goal setting, functional assessments, and charge entry following policies and regulatory guidelines
  • Participates in student programs, providing supervision and fostering learning opportunities
  • Creates an individualized professional development plan, pursues continuous education, and applies new knowledge to improve service quality
  • Adheres to State Practice Acts and maintains licensure in good standing
  • Understands and follows the organizations performance improvement initiatives, providing feedback to enhance service quality
