Job Summary:
Outpatient Physical Therapist specializing in outpatient physical therapy. This role involves evaluating patients, developing and implementing individualized treatment plans, and coordinating care to facilitate patient recovery and functional return to community activities. The position requires serving as an educator for patients, families, and colleagues, while adhering to safety, compliance, and professional standards to ensure high-quality care within the healthcare organization.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 0 years of outpatient clinical experience; CPR certification; valid applicable license for the state where work will be performed.
• Preferred Qualifications: Successful previous assignment with the agency; successful previous assignment with the organization.
• Additional Information: Performs comprehensive patient evaluations, develops treatment plans with clinical reasoning, and documents medical records accurately and timely in compliance with guidelines. Participates in student programs, provides professional development, adheres to state practice acts, and maintains licensure in good standing. Ensures care is provided within signed physician orders and insurance guidelines. Demonstrates commitment to performance improvement initiatives and safety protocols, including safe patient handling, infectious disease control, and slip/trip prevention.
