Job Summary
Pharmacist Inpatient, located in Ukiah, California, is responsible for verifying and filling medication orders, monitoring patient therapy, providing drug information, and ensuring medication safety. This professional supervises support staff and collaborates with healthcare teams to optimize medication management within a 50-bed acute care facility.

Job Details

• Position Description: Verify and fill medication orders, monitor patient medication therapies, provide drug information, identify and troubleshoot potential medication safety hazards, document all interactions for medical review, and supervise support personnel.
• Qualifications: Graduate of school of pharmacy (Doctor of Pharmacy degree preferred), pharmacist license in the state of practice (required), and all licensures and certifications needed to fulfill the duties of a pharmacist in the state (required).
• Certifications: Advanced Cardiovascular Life Support (ACLS) (required), Basic Life Support (BLS) (required), Pediatric Advanced Life Support (PALS) (required).
• Experience: Minimum of 1 year inpatient pharmacy experience preferred; minimum of 1 year technical experience preferred.
• Schedule: Rotating shifts, 10 hours per shift, with a minimum of 40 hours per week.
• Responsibilities: Process medication orders in compliance with policies and regulations, ensure efficacy and safety of medication therapy, participate in medication management activities, conduct daily medication utilization reports, perform quality assurance procedures, collaborate on medication procurement, and participate in process improvement initiatives.
