Outpatient Physical Therapist (PT OP) Located at the healthcare providers outpatient care setting, this position involves evaluating patients and developing tailored treatment plans to facilitate recovery and functional improvement. Job Details

• Position Description: Evaluates assigned patients, creating and adjusting treatment plans based on patient needs and physician directives. Responsible for delegating discipline-specific therapy services and designing functional treatment programs to enable a successful return to community activities.
• Certifications, Education, and Experience: Degree in Physical Therapy from an accredited school, CPR certification, and at least 1 year of outpatient clinical experience required. Active licensure in the state where work will be performed is mandatory.
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Additional Information: Completes comprehensive patient evaluations, documents in medical records accurately and timely, and provides supervision for student rotations. Seeks ongoing professional development and adheres to state practice acts and regulatory standards. Demonstrates commitment to organizational performance improvement and safety protocols, including safe patient handling and infection control.
