Job Summary
Physical Therapist (PT) Home Health ? Remote. This position involves providing outpatient physical therapy services in a home health setting. The role requires evaluating patients, developing treatment plans, and delivering therapy to improve patient outcomes. This assignment is structured for a short-term engagement, emphasizing bedside therapy and patient education.

Job Details

  • Certifications Required: BLS (Basic Life Support); Physical Therapy License.
  • Education Requirements: No specific degree required beyond licensure.
  • Location Requirements: Must have a current Driver's License and Auto Insurance.
  • Years of Experience: Minimum of 1 year of relevant experience preferred.
  • Schedule: Monday through Friday, 8:00 am to 5:00 pm, with 40 hours per week.
  • Additional Information:
    • No clinical involvement; position involves managing home health visits, recertifications, and start of care assessments.
    • The role involves regular visits, typically 5-6 per day, with adjustments for admissions or additional assessments.
    • Candidate may perform start of care, recertifications, and resumption of care.
    • May be required to work with physical therapists at sister agencies for consultation.
    • Short orientation period of 1-2 weeks with a preceptor.
    • Must adhere to the following dress code: Black, Navy blue, or grey scrubs.
    • Must pass a manager interview.
    • RTO (Request for Time Off) must be submitted upfront; additional RTO requests will not be approved.
    • Must be able to work within a 50-mile radius rule; candidates may not have been employed part-time, full-time, or PRN at facilities associated with the organization within the past 12 months.
    • No weekend shifts, and holidays off.
    • Position is primarily focused on home health services, with occasional hospice-related visits for safety evaluations or patient teaching.
