Job Summary
Physical Therapist Inpatient (PT IP) in the healthcare provider setting for the Yolo County area. This role involves providing physical therapy services to patients in a home health environment, assessing patient needs, developing treatment plans, and ensuring compliance with healthcare regulations. The position requires a dedicated professional capable of working independently and collaborating with a multidisciplinary team.

Job Details

• Schedule: Days shift, 8 hours per day, minimum 40 hours per week, starting 09/15/2026 and ending 01/12/2027
• Required Qualifications: Professional licensure as a Physical Therapist, BLS certification, a minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting, and the ability to work in the designated service area of Yolo County
• Responsibilities include assessing, evaluating, and updating patients rehabilitative status; developing and implementing treatment plans; instructing patients and caregivers; evaluating home environments; supervising Home Health Aides; documenting progress; and participating in case conferences and peer consultations
• Travel among service areas such as Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto is required, with mileage reimbursed at the GSA rate with prior approval
