Job Summary:
Physical Therapist Home Health (PT HH) in a home health care setting. This position involves providing physical therapy services to geriatric patients within a designated territory, ensuring quality care and adherence to clinical standards. The role requires flexibility with territory and timely documentation of patient progress.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Valid license in hand, minimum of 1 year of relevant experience, OASIS experience required for staff completing Start of Care (SOC), proficiency in Homecare Homebase (HCHB) required, no first-time travelers, timely communication and documentation mandatory.
• Preferred Qualifications: Experience with adult geriatric patients, home health care setting.
• Additional Information: All charting must be completed by 9 a.m. the following morning. The traveler is expected to be flexible with territoryup to a 60-mile radius from the hub or office. All travelers must have an active drivers license and personal transportation. On-call rotation may be required; discuss specifics during interview. All travelers are required to attend a Monday orientation.
• Submittal Requirements: Active driver's license, completion of modules during orientation, RTO (Requested Time Off) submitted at application, and compliance with trauma, drug screening, or other protocols as required.
