Job Summary
Pharmacist Inpatient, PHARM IP Located in an organizations healthcare facility, this position involves providing comprehensive pharmacy services within an inpatient setting. The role includes dispensing medications, monitoring drug therapies, and ensuring the safety and accuracy of patient medication records, supporting clinical care with efficiency and compassion.
Job Details
- Qualifications: Graduation from an ACPE-accredited School of Pharmacy with a Doctor of Pharmacy degree (preferred) or a BS Pharmacy degree (accepted); State licensure (LA license required); BLS or ACLS certification via AHA; hospital experience preferred; pharmacy registration in the state of practice.
- Schedule: Varying shifts, including 6:00am-2:30pm, 7:00am-3:30pm, 8:00am-4:30pm, 11:00am-7:00pm, and/or 3:00pm-11:00pm; 40 hours per week.
- Responsibilities include preparing and dispensing drug orders, transcribing and verifying orders in the computerized patient medication record, maintaining accurate drug records, compounding pharmaceuticals, issuing controlled substances, supervising pharmacy support personnel, and providing clinical consultation and education.
- Clinical duties encompass ensuring safe, appropriate, and cost-effective drug therapies, monitoring for contraindications, drug interactions, and allergies, assisting with pharmacokinetic and renal dosing, and participating in quality improvement activities.
- Additional responsibilities include participating in antimicrobial stewardship, opioid stewardship, and medication management activities, and attending interdisciplinary rounds.
- Submission Requirements: Candidates should fully meet the listed qualifications and shall be prepared to perform clinical and operational pharmacy duties.
