Job Summary
Physical Therapist Home Health (PT HH) Dover, DE. This position involves providing skilled physical therapy services to patients within a home health setting in accordance with physicians' plans of care. The role includes evaluating patients, developing treatment plans, and supervising therapy assistants as needed.

Job Details

• Certifications, Education, and Experience Requirements: Successful completion of a physical therapy program approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association; current and unencumbered physical therapy license specific to the state; 2-3 years of physical therapy experience preferred; current CPR certification.
• Schedule: Monday through Friday, 8:00 am to 5:00 pm, minimum of 40 hours per week.
• Responsibility and Duties: Provide skilled physical therapy services directly or through supervision, assist in evaluating patients' levels of function, develop and update treatment plans, observe and report patient progress, and coordinate discharge plans and assessments with therapy assistants.
• Additional Requirements: Must have reliable transportation, a current drivers license, and agency-required liability insurance. Proficiency in general computer operations, including internet, email, and electronic health record systems, is required.
