Job Summary
Outpatient Physical Therapist (PT OP) for a healthcare facility in the designated location. The role involves evaluating patients, developing and implementing treatment plans, and ensuring effective return to community activities. The position also includes serving as an educator for patients, families, and colleagues, while demonstrating professional standards and organizational values.
Job Details
- Complete a comprehensive evaluation of assigned patients.
- Utilize clinical reasoning to develop appropriate treatment plans and goals aimed at progressing patients toward discharge within the established length of stay.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in compliance with state, federal, and intermediary guidelines.
- Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient.
- Participate in student programs as assigned, providing a positive learning environment with appropriate supervision, development of learning objectives, and completion of evaluations.
- Actively create an individualized professional development plan with clinical management.
- Identify and initiate own professional growth, pursue continuing education, self-study, and involvement in professional activities and organizations.
- Apply learned information to meet the organizations strategic objectives.
- Adhere to discipline-specific State Practice Act and other regulatory agencies, maintaining licensure in good standing and completing all mandated continuing education requirements.
- Perform in an ethical manner aligned with professional codes of conduct.
- Demonstrate awareness of performance improvement initiatives and follow through with action plans.
