Job Summary
Physical Therapist (SNF) Massachusetts This position involves providing rehabilitative services to patients with physical functioning disorders within a skilled nursing facility. The role includes developing treatment plans, administering therapies, and evaluating patient progress to facilitate recovery and improved mobility.
Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: MA State License required; 1 year of physical therapist experience required; new graduates with Level II experience are acceptable; no minimum years of experience specified
- Schedule: Day shift, 8 hours per day, minimum of 40 hours per week
- Responsibilities include screening, evaluating, and rehabilitating patients with physical functioning disorders, wound care, and related treatments. Utilizes standardized assessments and evidence-based practices to support clinical interventions.
- Administers procedures such as manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care.
- Assesses patient progress and modifies treatments accordingly, documenting all care in the electronic health record system.
- Provides consultation to patients, families, and other providers regarding treatment plans and outcomes.
- Communicates effectively with the healthcare team regarding patient progress and barriers.
- Participates in meetings, training, and conferences as required.
- Ensures supervision and support of physical therapist assistants, rehabilitation aides, and students when applicable.
- Adheres to the American Physical Therapy Association (APTA) and state practice acts, ethics, and conduct standards throughout professional practice.
- Submission requirements include verifying experience and licensure as outlined.
