Physical Therapist Home Health (PT HH) Arlington, Everett, Malden, Medford, West Medford, Melrose, Somerville. This position involves providing specialized physical therapy services to patients in their residences who have physical impairments, functional limitations, or disabilities due to injury, illness, or other causes. The role requires collaboration with patients, families, physicians, and multidisciplinary teams to develop and implement care plans, ensuring optimal patient outcomes.
Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Experience: Massachusetts Physical Therapist Professional license; at least 1 year of home care and travel experience; ability to travel to assigned cases.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Responsibilities include delivering physical therapy services in a home setting, documenting care accurately and promptly via electronic health records, and coordinating care with the interdisciplinary team.
