Job Summary
Physical Therapist Outpatient (PT OP) The outpatient physical therapist will evaluate and develop treatment plans for patients, focusing on functional recovery and community reintegration. This role involves delegation of therapy services, patient education, and ensuring compliance with organizational standards within a healthcare facility setting.

Job Details

• Complete a comprehensive evaluation of assigned patients and develop, implement, and modify treatment plans based on patient needs, goals, and physician direction.
• Perform medical record documentation accurately, concisely, and in a timely manner, including goal setting, quality assessments, and charge entry in accordance with state, federal, and intermediary guidelines.
• Ensure care is provided under signed, valid physician orders and within insurance approvals as necessary for clinical care.
• Participate in student programs to foster a positive learning environment, providing appropriate supervision and completing evaluations.
• Actively pursue professional development through continuing education, self-study, and organizational involvement, applying learned information to meet organizational goals.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing, completing all mandated continuing education requirements.
• Understand and apply the organization's performance improvement methodology, providing feedback to enhance service quality.
• Demonstrates behaviors and attitudes aligned with organizational Mission Statement and Core Values, promoting safety and professionalism at all times.
• Schedule: Days, 8-hour shifts, 40 hours per week.
