Physical Therapist Home Health (PT HH) This contract assignment is based in the Berkshire County area. The role involves providing skilled physical therapy services in a home health setting, following physicians' orders, and supporting patients' recovery and mobility. Job Details

• Position Description: Performs patient evaluations and provides therapeutic services according to physicians' orders. Responsible to the Director of Operations / Clinical Manager. Directly supervises and evaluates Physical Therapy Assistants and Home Health Aides as assigned.
• Required Qualifications: Current and unencumbered license to practice physical therapy specific to the state of assignment; successful completion of a physical therapy program approved by the American Physical Therapy Association or equivalent; 2-3 years of physical therapy experience preferred; current CPR certification; familiarity with general computer use, including internet, email, and online services.
• Schedule: Days, from 8 am to 5 pm, 40 hours per week.
• Additional Information: Provides physical therapy services in accordance with the physicians' plan of care in a home health environment. Must have reliable transportation, a current drivers license, and agency-required liability insurance. Periodic on-site assessments, reevaluations, and coordination of discharge plans are required.
