Job Summary
Outpatient Physical Therapist (PT OP) Location: Not Specified. This position involves evaluating assigned patients, developing and modifying treatment plans to meet individual needs and physician directives. The role includes delegating therapy services, designing functional treatment programs for effective community reintegration, and serving as an educator for patients, families, and colleagues.

Job Details

• Position involves conducting comprehensive evaluations of assigned patients and utilizing clinical reasoning to create appropriate treatment plans with goals focused on progressing patients toward discharge within an established length of stay.
• Completes medical record documentation accurately, including goal writing, functional assessment, letters of medical necessity, and charge entry, in compliance with state, federal, and intermediary guidelines.
• Ensures patient care is provided under signed, valid physician orders and insurance approval, as necessary for clinical care.
• Participates in student programs by providing supervision, developing learning objectives, and completing evaluations to foster a positive learning environment.
• Maintains professional licensure in good standing and fulfills all state-mandated continuing education requirements.
• Applies learned information to meet organizational strategic goals and actively pursues professional development through continuing education, self-study, and professional involvement.
• Follows all clinic guidelines regarding personal appearance, attendance, and punctuality, and adheres to safety policies focused on patient and employee safety, including safe patient handling and infection control practices.
• Demonstrates organizational awareness of performance improvement initiatives, offering feedback to help improve service quality.
