Job Summary
Physical Therapist Outpatient (PT OP) Location: not specified. This outpatient clinical position involves evaluating patients, developing and modifying treatment plans, and ensuring effective patient recovery in a community setting. The role emphasizes patient safety, clinical excellence, and adherence to regulatory standards to support optimal patient outcomes.

Job Details

• Completes comprehensive evaluations of assigned patients, utilizing clinical reasoning to develop appropriate treatment plans focused on progressing patients toward discharge within an established length of stay.
• Utilizes clinical reasoning to develop appropriate treatment plan and goals focused on progressing patients towards discharge within an established length of stay.
• Completes medical record documentation in a clear, concise, accurate, and timely manner, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in compliance with state, federal, and intermediary guidelines and in accordance with the clinic's policies and procedures.
• Ensures care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient.
• Participates in student programs as assigned and appropriate, providing a positive learning environment with appropriate supervision, development of learning objectives, and completion of student evaluations.
• Applies learned information to meet the strategic objectives of the organization.
• Adheres to discipline-specific State Practice Acts and other regulatory agencies. Maintains professional licensure in good standing and completes all state-mandated continuing education requirements.
• Performs in an ethical manner in accordance with discipline-specific professional codes of conduct.
• Understands the organizations performance improvement methodology to improve service quality, offering feedback to maximize performance outcomes.
• Promotes patient and employee safety, focusing on a 10 Foot Circle of Safety and displaying safety awareness in key areas.
