Physical Therapist Home Health (Part-Time) The organization is seeking a licensed physical therapist to provide home health services. Job Details

• Position Description: No clinical involvement outside of patient visits; profiles sent directly to the manager for phone interview. Must have home care home base experience and home health experience.
• Certifications and Education Requirements: BLS (American Heart Association) certification; valid license in hand at the time of submission.
• Responsibilities: Conduct start of care, recertifications, evaluations, and daily visits, typically 6.5 points per day/week.
• Schedule: Monday through Friday, 8:00 am to 5:00 pm, with rotating weekend shifts. Shifts generally last 5-8 hours, with a minimum of 40 hours per week.
• Preferred Qualifications: No specific preferred qualifications listed; experience in home health care preferred but not explicitly required for minimal qualifications.
• Additional Information: Two-week orientation with a preceptor, including a few visits with a work coach. A clinical manager or educator is available 24/7 for emergencies or during 8-5 office hours. The job involves using Homecare Homebase (HCHB) charting system. Travel and tablet will be provided, and there is a rotating holiday schedule.
