Physical Therapist Home Health in a regional healthcare setting. This role involves providing specialized home health services, focusing on geriatric patients, with an emphasis on timely documentation and effective communication. Job Details

• Location: Up to a 60-mile radius from the healthcare providers hub/office.
• Position: Physical Therapist (Home Health)
• Certifications, Education, and Experience: Valid license in hand, minimum of 1 year of relevant experience, and recent work in home health settings.
• Schedule: 8-hour shifts, 40 hours per week, starting on 10/26/2026 and ending on 01/25/2027.
• Patient Population: Geriatric patients with diagnoses including post-operative cervical, shoulder, and spinal surgeries, as well as neurological conditions such as Multiple Sclerosis and Post Stroke Rehabilitation.
• Responsibilities: Deliver skilled therapy services, manage approximately 5-7 patients daily, ensure timely documentation, and adhere to departmental protocols.
• Additional Operational Details: Timely communication and documentation are mandatory. An active drivers license and personal transportation are required. RTO requests should be submitted at the time of application.
• Previous experience with OASIS is required for staff completing Start of Care assessments. Proficiency in Homecare Homebase (HCHB) is mandatory.
• On-call rotation may be required and will be discussed during the interview.
• First-time travelers are not accepted.
