Occupational Therapist Home Health (OT HH) This position is based in a healthcare organization providing home health services. The role involves delivering occupational therapy to geriatric patients within a specified territory, ensuring comprehensive patient care and timely documentation. The position requires a licensed professional committed to providing high-quality services in a home health setting.
Job Details
- Position: Occupational Therapist Home Health
- Location: Territory within up to a 60-mile radius from the hub or office
- Certifications: BLS (Basic Life Support), NBCOT (National Board for Certification in Occupational Therapy)
- Education Requirements: Active license in hand
- Experience Requirements: Minimum of 2 years of relevant experience, with 1 year of home health or clinical experience preferred; OASIS experience REQUIRED for staff completing Start of Care (SOC) in home health; proficiency in HCHB (Homecare Homebase) REQUIRED
- Schedule: Days shift, 8 hours per day, minimum 40 hours per week
- Additional Information:
- Prior home health, acute care, or outpatient experience preferred
- Must be flexible with territory and be able to work independently
- On-call rotation may be required; discussed during interview
- Timely documentation and communication are mandatory
- Active drivers license and personal transportation required
- RTO (Requested Time Off) must be submitted at the time of application
- Modules are billable and completed during orientation
- Former permanent employees must be separated for at least 6 months before returning as travelers
- Travel Pairs accepted, but each must have their own territory and transportation
