Job Summary
Physical Therapist Home Health (PT HH) This position is based in an identified geographic area within a 60-mile radius of the hub. The role involves providing outpatient therapy services to geriatric patients in a home health setting, focusing on improving patient mobility and function. The position supports a dynamic team committed to delivering patient-centered care.
Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid state license in hand, BLS certification, a minimum of 1 year of relevant experience, including OASIS experience for staff completing Start of Care (SOC), proficiency in Homecare Homebase (HCHB), and flexibility with territory.
- Schedule: Monday through Friday, 8-hour shifts, minimum of 40 hours per week.
- Additional Information:
- Home health experience is required.
- RTO (Requested Time Off) must be submitted at the time of application.
- On-call rotation may be required, to be discussed during interview.
- Timely communication and documentation are mandatory.
- All travelers must have an active driver's license and personal transportation.
- Orientation begins on Monday.
- Modules are billable and completed during orientation.
- Previous travel experience is not required; first-time travelers are accepted.
- No dress code restrictions besides scrubs in any color, no jeans.
