Description
Job Summary:
Physical Therapist specializing in Home Health will provide outpatient therapy services within a designated territory. The role involves delivering specialized care to geriatric patients, performing timely documentation, and adhering to all clinical protocols. The position is scheduled for day shifts and requires flexibility to cover assigned territory.
Job Details:
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Valid license in-hand, active drivers license, minimum 0 years of experience, proficiency in HCHB (Homecare Homebase), OASIS experience for staff completing Start of Care (SOC), and strong knowledge in outpatient therapy principles.
- Preferred Qualifications: Previous experience in home health or outpatient settings, familiarity with a variety of patient diagnoses, and experience with interdisciplinary teams.
- Additional Information:
- Home Health only; travel Pairs accepted but each must have own transportation.
- Must be flexible with territory covering up to a 60-mile radius from the designated hub or office.
- Timely communication and documentation are mandatory.
- Training includes orientation on Monday.
- On-call rotation may be required; to be discussed during interview.
- All travelers are required to have an active drivers license and personal transportation.
- RTO (Requested Time Off) must be submitted at the time of application.
- Modules are billable and completed during orientation.
- Former permanent employees must be separated for a minimum of 6 months before returning as travelers.
