Job Summary
Physical Therapist (PT) Home Health This opportunity is located within the healthcare provider's home health setting. The role involves providing expert physical therapy services to geriatric patients, supporting recovery and functional improvement through personalized treatment plans and patient education.
Job Details
- Certifications: BLS, active license in-hand, with submission required
- Education Requirements: Not specified
- Years of Experience: Minimum of 2 years of relevant experience, with at least 1 year in a home health setting, including OASIS experience for staff completing Start of Care (SOC)
- Schedule: Days shift, 8 hours per day, minimum 40 hours per week
- Required Qualifications: Valid license in current state, minimum of 2 years of relevant experience, recent experience in geriatric care, proficiency in OASIS and Homecare Homebase (HCHB), timely communication, and documentation
- Additional Information: Requires flexibility with territory within up to a 60-mile radius, active drivers license and personal transportation, and submission of RTO (Requested Time Off) at application. On-call rotation may be required.
- Submission Requirements: All travelers must have an active drivers license, and personal transportation is mandatory.
