Description
Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in the local area. This role involves providing skilled therapy care to patients in their homes, following all clinical guidelines and safety protocols. The position requires delivering services in accordance with the established treatment plan and collaborating with a multidisciplinary team to optimize patient outcomes.
Job Details:
- Schedule: 8-hour days shift, with a minimum of 40 hours per week from 10/05/2026 to 01/04/2027.
- Required Qualifications: Valid state license (compact license preferred), at least 1 year of skilled home health experience, BLS certification.
- Preferred Qualifications: Home health experience is highly preferred.
- Certifications: BLS required.
- Additional Information:
- COVID-19 vaccination series and booster are not required.
- Candidate is expected to have reliable transportation; occasional travel within a 60-mile radius is required.
- System uses electronic charting and documentation via EPIC and Rover applications.
- Candidates may be considered as local if they reside within a 50-75 mile radius.
- No float requirement is specified for this assignment.
- Must commit to working three of the five holidays, with all scheduling requests disclosed upfront during submission.
