Job Summary:
Home Health Physical Therapist in the Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington, Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Vallejo, Benicia, Napa, American Canyon, or Yountville areas. This position involves providing skilled therapy services in a home setting, with potential travel outside the designated service areas and weekend coverage if required. The role requires adherence to home health standards and the ability to work independently in a community-based environment.

Job Details:

• Schedule: Days, 9 hours per shift, minimum of 36 hours per week
• Required Qualifications: Current California Physical Therapist license, BLS certification, valid drivers license, auto insurance and registration, TB test (annual), MMR, Varicella, HepB, TDaP (last 10 years), Flu (annual), drug screening, physical exam. No minimum years of experience required.
• Preferred Qualifications: Experience in home health or outpatient therapy settings is preferred.
• Additional Information: The position includes performing a variety of therapy sessions and assessments, including OASIS components such as ReCertification, Routine, Evaluation, Discharge, and subsequent visit types. Pay is based on per visit basis, with all-inclusive rates covering drive, visit, documentation, and communication time. Candidate must be willing to travel outside the designated areas or work weekends if necessary. All requested time off must be disclosed at the time of submission; post-acceptance RTO requests will not be approved.
