Job Summary
Physical Therapist (SNF) specializing in skilled nursing facilities is needed in the designated service areas. This role involves providing therapy services to patients in various settings, with a focus on maintaining and improving patient mobility and function. The position offers flexibility to travel outside the immediate area and may require weekend coverage to meet patient care needs.

Job Details

• Schedule: Day shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Current California Physical Therapist (PT) license; BLS certification; valid drivers license; auto insurance and registration; annual TB test; MMR, Varicella, Hepatitis B, Tdap (valid within 10 years), annual flu vaccination; recent drug screening and physical exam; no minimum experience required
• Additional Information: Travel outside the designated service area may be necessary. Responsibilities include completing visits and documentation as specified, with time approved on a per visit basis covering travel, visit, documentation, and communication activities. Flexibility for weekend coverage and working with COVID-positive and PUI (Persons Under Investigation) patients is required. All requested time off must be disclosed at the time of submission and cannot be added post-acceptance.
