Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Experience: BLS certification, active license in hand, minimum of 1 year of relevant experience, recent experience preferred, no experience requirement for first-time travelers.
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Minimum of 1 year of relevant experience; OASIS experience REQUIRED for staff completing SOC (Start of Care) Home Health ONLY; Proficiency in Homecare Homebase (HCHB) REQUIRED; active license in hand; Monday orientation required; all travelers must have an active drivers license and personal transportation.
• Additional Information: The role involves providing care to geriatric patients with 5-7 patient visits daily within up to a 60-mile radius from the hub/office. On-call rotation may be required, and timely documentation and communication are mandatory. Travelers must be flexible with the territory and adhere to the dress code of scrubs, any color, with no jeans. Modules are billable and to be completed during orientation. RTO (Requested Time Off) must be submitted at the time of application.
