Job Summary:
Physical Therapist Outpatient (PT OP) position available in a healthcare facility. This outpatient therapy role involves evaluating patients, developing individualized treatment plans, and working toward patient discharge goals. The position emphasizes patient safety, professional development, and adherence to clinical standards within an outpatient setting.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active and applicable license for the state where work will be performed; ability to complete a prior assignment with the agency or organization (highly desired)
• Preferred Qualifications: Prior successful assignment with the agency or organization (nice to have)
• Additional Information: Evaluates all assigned patients and develops, implements, and modifies treatment plans based on patient needs and physician direction. Responsible for delegation and supervision of all discipline-specific therapy services for assigned patients. Ensures the design of functional treatment programs to support effective community reintegration. Performs documentation in accordance with regulatory and organizational policies, including goal writing, medical necessity letters, and charge entry. Participates in student programs, providing supervision and development opportunities. Maintains licensure and adheres to professional standards and organizational policies. Demonstrates safe practices in patient handling, infection control, and work environment safety. Engages in continuous professional development through ongoing education and involvement in professional organizations. Follows the organizations performance improvement initiatives and contributes to quality enhancement efforts.
