Job Summary
Physical Therapist (Home Health) in a home health setting. This position involves performing patient evaluations and providing therapeutic services according to physicians' orders. The role requires assessing patients in their home environment, developing treatment plans, and coordinating care to support patient recovery and mobility. The position is a temporary assignment with a focus on delivering quality therapy services directly to patients in their homes.

Job Details

• Schedule: Days, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Current and unencumbered license to practice physical therapy specific to the state. Successful completion of a physical therapy program at a school approved by the American Physical Therapy Association or an equivalent authority. 0+ years of experience preferred. Current CPR certification. Familiarity with general computer use, including email, internet navigation, and online HR and training platforms.
• Preferred Qualifications: 2-3 years of physical therapy experience in a home health setting. Previous experience working with electronic medical records such as HomeCare HomeBase, Oasis, and Outlook.
• Additional Information: The role involves conducting home visits, participating in in-service education programs, and coordinating discharge and assessment plans. Reliable transportation, a current drivers license, and liability insurance are required.
• Start and End Dates: 10/05/2026 to 01/03/2027.
• Travel: Expect to travel approximately 30-40 miles per day for home visits.
• On-Call: One Saturday per month.
