Job Summary
Physical Therapist (Outpatient) Montana. This position involves providing skilled physical therapy services to Home Health and Hospice patients in their homes. Responsibilities include conducting comprehensive patient evaluations, developing treatment plans with both short-term and long-term goals, and implementing interventions such as therapeutic exercises, mobility training, and balance activities. The therapist will also assess home safety, educate patients and caregivers, monitor progress, and collaborate with other healthcare professionals to ensure optimal patient outcomes.

Job Details

• Position Description: Provide physical therapy services including evaluations, treatment planning, therapeutic interventions, patient education, and safety assessments in a home health setting.
• Certifications, Education, and Experience Requirements: Current Physical Therapist (PT) licensure in Montana, valid drivers license, BLS certification, and at least 0 years of experience.
• Schedule: Days shift, 12 hours per day, minimum of 36 hours per week.
• Additional Information: Candidates will travel to patient homes and other care settings to deliver therapy services. Effective communication and collaboration with healthcare team members are essential for success.
