Job Summary
Physical Therapist Outpatient in a healthcare setting located in the outpatient care environment. This position involves evaluating patients, developing and implementing individualized treatment plans, and providing therapy services aimed at facilitating functional return to community activities. The role includes patient education and adherence to organizational standards of safety and professional conduct.

Job Details

• Position Description: Evaluates assigned patients, develops, implements, and modifies treatment plans according to patient needs, goals, and physician directives. Responsible for delegation and direction of therapy services for designated patients, ensuring the design of effective treatment programs. Serves as an educator for patients, families, students, and colleagues, and performs additional professional duties related to patient care and program development. Demonstrates compliance with the organization's Mission Statement and Core Values, including safety and confidentiality protocols.
• Certifications, Education, and Experience: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR Certification; valid and active license for the state where work will be performed; minimum of 0 years experience required.
• Schedule: Dayshift; 8 hours per day; minimum of 40 hours per week, starting October 12, 2026, through January 11, 2027.
• Additional Responsibilities: Completes comprehensive patient evaluations, utilizes clinical reasoning to develop appropriate treatment goals, and documents medical records accurately and timely. Participates in student programs, offers professional development initiatives, adheres to state practice acts and regulatory standards, and contributes to performance improvement efforts.
