Job Summary
Physical Therapist Outpatient (PT OP) Location: not specified. This outpatient position involves evaluating and developing treatment plans for assigned patients, focusing on effective return to community activities. Job Details

• Certifications, Education, and Experience: Degree in Physical Therapy from an Accredited School; CPR Certification; 6 months to 1 year of outpatient clinical experience; 6 months preferred; no minimum years of experience required
• Schedule: Shift hours are 8 hours per day, with a minimum of 40 hours per week
• Completes comprehensive evaluations and develops individualized treatment plans based on clinical reasoning emphasizing patient discharge goals within the established length of stay
• Performs timely and accurate medical record documentation, including goal writing, functional assessments, and necessary forms, following all regulatory and organizational policies
• Ensures all care is provided under valid physician orders and insurance approvals as required for clinical appropriateness
• Participates in student training programs, providing supervision and fostering a positive learning environment
• Initiates and pursues ongoing professional development through continuing education and involvement in professional organizations
• Adheres to state practice acts and maintains licensure in good standing, completing all required continuing education
• Demonstrates commitment to the organizations performance improvement initiatives, providing feedback and participating in quality enhancement efforts
