Job Summary
The healthcare organization is seeking a Physical Therapist (Inpatient) in a home health setting. This role involves providing physical therapy services to patients based on physicians plans of care, including assessment, evaluation, and treatment in various territories within Yolo County.

Job Details

• Position: Physical Therapist (Inpatient) Home Health
• Location: Yolo County, including Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, Esparto
• Schedule: Days, 8-hour shifts, minimum 40 hours per week, with rotations on Saturdays approximately once a month
• Required Qualifications: Minimum of 2 years experience as a physical therapist in an acute, home care, or rehabilitation setting; BLS certification; valid license to practice; no specific experience requirement listed otherwise.
• Responsibilities:
  • Assessing, evaluating, and updating therapeutic, rehabilitative, and functional status of patients
  • Directing physical therapy treatment and instructing patients and families in the use and care of therapeutic appliances
  • Evaluating home environments and assessing muscle strength, mobility, gait, and range of motion
  • Reporting patient reactions and changes to physicians
  • Initiating therapy programs and mentoring other personnel and family caregivers
  • Supervising Homemaker/Home Health Aides participating in patient rehabilitation
  • Identifying additional home health needs and referring as necessary
  • Submitting progress summaries and reports on patient's condition
  • Providing consultation to home caregivers and participating in peer review and staff assessments
  • Ensuring compliance with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and licensure regulations
  • Performing other duties as assigned
