Job Summary
Physical Therapist Home Health (PT HH) The organization is seeking a qualified Physical Therapist to provide home health services within a designated territory. The role involves delivering patient-centered care, completing timely documentation, and adhering to clinical and operational standards in a home health setting.

Job Details

• Certifications and Qualifications: BLS certification, active license in hand, minimum of 1 year of relevant experience, proficiency in Homecare Homebase (HCHB), and OASIS experience required for staff completing Start of Care (SOC).
• Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
• Responsibilities: Deliver home health Physical Therapy services, ensure timely charting by 9 a.m. the following morning, participate in on-call rotations, and provide patient education.
• Additional Requirements: Flexibility with territory, active drivers license, personal transportation, and submission of requested time off at application. All travelers must have an active drivers license, and transportation is required.
• Preferred Qualifications: Experience in outpatient clinics, rehabilitation hospitals, or skilled nursing facilities.
• Submission Requirements: Candidates must be licensed, with documentation to be submitted upon application, and attend a Monday orientation.
