Job Summary
Physical Therapist Outpatient (PT OP) Located in an outpatient setting, this role involves evaluating patients, developing, implementing, and modifying personalized treatment plans to support functional recovery and community reintegration. The position emphasizes patient education, interdisciplinary collaboration, and adherence to quality and safety standards.

Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an accredited school; 2 years of outpatient clinical experience preferred; CPR certification.
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Completes comprehensive patient evaluations and develops appropriate treatment goals focused on discharge planning within established length of stay.
• Performs accurate and timely documentation including goal setting, assessments, letters of medical necessity, and charge entry, all in compliance with regulatory standards.
• Provides supervision and mentorship within student programs, creating positive learning environments and evaluating student progress.
• Engages in ongoing professional development through continuing education, self-study, and involvement in professional organizations.
• Ensures care delivery complies with state practice acts and regulatory standards, maintaining licensure in good standing.
• Participates in performance improvement initiatives, providing feedback to enhance service quality and safety.
• Demonstrates behaviors consistent with organizational values, emphasizing safety, teamwork, results orientation, and professional integrity.
• Completes assignments between 10/19/2026 and 01/18/2027.
