Job Summary
Speech Language Pathologist (SNF) Massachusetts. This position involves providing specialized speech therapy services to patients in a skilled nursing facility setting. The role focuses on evaluating, rehabilitating, and supporting patients with communication, cognitive, or swallowing disorders, ensuring standards of regulatory and clinical practice are met.
Job Details
- Required Qualifications: Massachusetts State License; CCC-certified Speech-Language Pathologist (SLP); 1 year of experience in speech-language pathology; experience in Long-Term Care (LTC) or rehab settings; familiarity with ASHA, State practice acts, and ethics.
- Schedule: Days shift, 8 hours daily, with a minimum of 40 hours per week.
- Able to supervise and support CFY or Student Speech-Language Pathologists.
- Effectively screens/evaluates/rehabilitates patients with communication, cognitive, or swallowing disorders and develops appropriate plans of care following all regulatory and clinical practice standards.
- Utilizes standardized assessments and evidence-based practices to support clinical interventions.
- Assesses patient progress and adjusts treatments accordingly according to clinical documentation requirements.
- Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation and referrals.
- Provides consultation and counseling to patients, families, caregivers, and other providers related to speech disorders.
- Communicates effectively with the clinical team regarding patient progress, barriers, and treatment plans.
- Documents all required clinical and regulatory information in the electronic documentation system according to policy.
- Accurately records treatment minutes for services provided, in compliance with regulatory standards.
- Participates in patient care conferences, utilization review meetings, rehabilitation meetings, and mandatory trainings or in-services.
- Able to obtain any necessary Durable Medical Equipment (DME) or supplies per clinical requirements.
