Job Summary
Physical Therapist Home Health (PT HH) This position is based in a healthcare setting specializing in home health services. The role involves providing rehabilitative therapy to patients in their residences, ensuring personalized and effective care aimed at optimizing patient outcomes during the specified contract period starting November 1, 2026, through January 31, 2027.

Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid state licensure as a Physical Therapist required; 0 years of experience minimum.
• Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Minimum 1 year of relevant experience, OASIS experience required for staff completing Start of Care (SOC), proficiency in Homecare Homebase (HCHB) required.
• Preferred Qualifications: None specified.
• Float: Float rotation may be required.
• Additional Information: No First Time Travelers, timely communication and documentation are mandatory. Active drivers license and personal transportation are required. RTO requests must be submitted at application, and on-call rotation may be required.
• Submission Requirements: All travelers must submit licensure documentation, and be available for Monday orientation. Former employees must be separated for a minimum of 6 months before returning as travelers and must be approved by HR. Travel pairs accepted, but each traveler must have their own transportation and territory assignment. Modules are billable and completed during orientation.
