Job Summary
Physical Therapist Home Health (PT HH) Seaside, OR, and surrounding areas. This position involves providing skilled physical therapy services in a home health setting. The role requires evaluating patients, developing care plans, and delivering therapy to improve patient outcomes in a community-based environment.
Job Details
- Certifications/Education/Experience/Location/Years of Experience: BCLS certification required; active drivers license and personal transportation essential; minimum of 1 year of relevant home health experience; experience with HCHB (Homecare Homebase) and Oasis systems required.
- Schedule: Days shift, 8 hours per day, 40 hours per week.
- Required Qualifications: Valid license in hand; at least 1 year of skilled home health experience; experience in home health OASIS documentation; proficiency in HCHB system; timely communication and documentation; active drivers license and personal transportation; RTO (Requested Time Off) must be submitted at application; Monday orientation.
- Additional Information: Travel involves occasional drives within approximately 60 miles per day, with the need for own transportation. The candidate should be prepared to work in a home health environment with interdisciplinary support, serving a variety of patient diagnoses.
