Job Summary
Physical Therapist Home Health (PT HH) Medford, OR and Surrounding Areas. This position involves providing skilled physical therapy services to patients in their homes, focusing on rehabilitation and functional improvement. The role demands delivering quality care while managing a caseload within specified patient ratios.

Job Details

  • Location: Medford, OR and Surrounding Areas
  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
  • Required Qualifications: Valid license in-hand, minimum of 1 year of relevant experience in home health, proficiency in HCHB (Homecare Homebase), current BCLS certification, active drivers license, and personal transportation.
  • Preferred Qualifications: Experience with OASIS (Outcome and Assessment Information Set) completion for Start of Care (SOC), and familiarity with home health documentation and protocols.
  • Responsibilities: Deliver comprehensive physical therapy services, complete OASIS assessments for SOC, document patient progress accurately and timely, and adhere to company policies and state regulations.
  • Certifications: BCLS required
  • Additional Information:
    Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles. Facility requires primary series COVID vaccination in addition to one booster.
    This role requires timely communication and documentation. On-call rotation may be requireddiscuss during interview.
