Job Details

• Position Description: Home Health Physical Therapist responsible for delivering outpatient therapy services in patients' homes in the specified geographic area, adhering to clinical and organizational standards.
• Required Qualifications: Valid license in the state of Oregon, BCLS certification, minimum of 1 year of skilled home health experience, proficiency in Homecare Homebase (HCHB), on Mondays orientation, active driver's license, and personal transportation.
• Schedule: 8-hour days, Monday through Friday, with a minimum of 40 hours per week.
• Open to: First-time travelers, with all applicants required to submit RTO (Requested Time Off) at the time of application.
• Additional Information: The traveler should have their own transportation, as occasional maximum drive radius is roughly 60 miles per day. System works to minimize drive and windshield time. Travel Pairs are accepted but each must have their own territory and transportation. Modules are billable and completed during orientation.
• Submission Requirements: License in-hand required with submission, timely communication and documentation mandatory, and all travel compliance requirements followed.
