Job Summary
Physical Therapist Assistant (Home Health) This opportunity is based in an outpatient home health setting within an undefined specialty. The position involves providing therapeutic services to adult and geriatric patients, with a focus on post-operative care, rehabilitation, and chronic condition management. The work entails delivering patient-centered care while ensuring compliance with all relevant clinical guidelines and documentation standards.

Job Details

• Certifications and Qualifications: BLS certification; active drivers license; minimum of 1 year relevant experience; proficiency in HCHB (Homecare Homebase) required; OASIS experience required for staff completing Start of Care (SOC) in home health; recent experience with 6+ patient visits daily or a minimum of 30 points weekly is preferred.
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Additional responsibilities: Timely communication and documentation required; familiarity with interdisciplinary support including physical therapy, interpretation services, and social services.
• Additional Notes: On-call rotation may be required; RTO (Requested Time Off) must be submitted at the time of application; travel Pairs accepted but each traveler must have their own transportation; modules are billable and completed during orientation; license in hand required upon submission; all travelers need active drivers licenses and personal transportation.
• Submission Requirements: Must submit all required documentation including active license, communication skills, and transportation details; RTO requests should be submitted proactively at application.
