Job Summary
Physical Therapist (Home Health) This position is based in an outpatient home health setting, working within a defined territory and supporting adult and geriatric patients. The role involves delivering specialized therapy interventions to enhance patient recovery and independence, with an emphasis on timely documentation and communication.

Job Details

• Position: Physical Therapist (Home Health)
• Specialty: Home Health
• Location: Up to a 60-mile radius from the hub/office; locals accepted and at travel rate
• Shift: Days, 8 hours per shift, with a minimum of 40 hours per week
• Certifications and Education: Current BLS certification, active drivers license in hand, and proficiency in HCHB (Homecare Homebase) are required
• Experience: Minimum of 1 year of relevant experience in home health, OASIS experience required for staff completing Start of Care (SOC); recent experience preferred
• Additional Requirements: Timely communication and documentation are mandatory; RTO requests must be submitted at the time of application; on-call rotation may be required
• Responsibilities: Providing comprehensive physical therapy assessments and interventions, utilizing interdisciplinary resources, and supporting patients with a variety of diagnoses and post-operative conditions
• Submission Requirements: All candidates must have an active drivers license, transport themselves independently, and have completed Monday orientation. Former employees must be separated for at least 6 months before reapplying as travelers.
