Job Summary
The organization is seeking a Physical Therapist Outpatient (PT OP) in a community-based setting. The role involves evaluating patients, developing and implementing treatment plans, and ensuring safe and effective patient care with adherence to professional standards and organizational values.

Job Details

• Position Description:
  Evaluates all assigned patients, develops, implements, and modifies treatment plans according to patient needs, goals, and physician directions. Responsible for delegation and supervision of therapy services, designing functional programs to promote patient reintegration into community activities. Serves as an educator for patients, families, students, and colleagues, while maintaining compliance with organizational standards and promoting safety practices.

• Required Qualifications:
  Degree in Physical Therapy from an accredited school, CPR certification, active state license, 6 months to 1+ year of outpatient clinical experience, and experience with regulatory requirements for practice.

• Schedule:
  Days shift, 8 hours per day, minimum of 40 hours per week.

• Additional Information:
  Completes comprehensive evaluations, documents medical records clearly and timely, ensures care aligns with signed physician orders and insurance approvals, participates in student training, pursues ongoing professional development, and adheres to state regulations and licensure requirements.
