Job Summary
Physical Therapist Home Health Missoula, MT. This position involves providing evaluation and rehabilitation interventions in patients' homes within the service area. The role includes managing and supervising patient care, delivering education to patients and their families, and collaborating with multidisciplinary teams to meet individual care needs.

Job Details

  • Location: Missoula, MT requires travel to patients' homes within the service area.
  • Schedule: Typically Monday through Friday, 8:00 am to 5:00 pm, with occasional evening or weekend coverage.
  • Qualifications include BS degree or higher from an accredited Physical Therapy program, current licensure to practice in the State of Montana, valid drivers license, reliable transportation, and appropriate auto insurance. Two years of previous Physical Therapist experience is preferred.
  • Schedule: 8 hours per day, minimum 40 hours per week.
  • Provides direct patient care in the home setting based on physicians' orders and established care plans.
  • Evaluates patients, develops and adjusts care plans, and applies evidence-based practice guidelines while adhering to Medicare regulations and Conditions of Participation.
  • Manages documentation of all skilled services, adverse events, and communications within the medical record.
  • Participates in multidisciplinary conferences and quality improvement initiatives.
  • Supervises Physical Therapist Assistant and performs supervisory visits within the required time frame.
  • Ensures compliance with licensure standards, including continuing education.
  • Communicates effectively with physicians and team members, and collaborates to ensure optimal patient care.
  • Takes part in relevant care conferences, manages patient admissions, discharges, and OASIS timing throughout the care episode.
  • Maintains accurate time logs and mileage, following agency policies for start/end of day commuting.
  • Communicates daily with the scheduler to maximize continuity of care and optimize travel routes.
