Job Summary
Occupational Therapist Home Health (OT HH) a healthcare provider in a community setting. This position involves delivering occupational therapy services to geriatric patients within a home health environment, with a focus on improving patient function and quality of life through specialized interventions.

Job Details

  • Position: Occupational Therapist Home Health (OT HH)
  • Schedule: Monday - Friday, with flexibility to work weekends; 8-hour shifts, minimum of 40 hours per week
  • Location Requirements: Up to a 60-mile radius from the hub/office; local candidates accepted and travel rate provided
  • Certifications, Education, and Experience: BLS certification; current license in hand; minimum of 1 year of relevant experience; recent experience in home health is required; proficiency in HCHB (Homecare Homebase) is required; OASIS experience is mandatory for staff completing Start of Care (SOC); no first-time travelers; timely communication and documentation required
  • Additional Requirements: Experience with acute care, activities of daily living, biofeedback, electrical stimulation, and Orthoplast, serial/inhibitory and static splints preferred; active drivers license and personal transportation required; RTO (Requested Time Off) must be submitted at the time of application; on-call rotation may be required; must complete Monday orientation.
  • Responsibilities: Providing occupational therapy services, including assessments, treatment plans, and patient education to geriatric patients in a home health setting; documentation and communication as per facility standards; adhering to dress code in scrubs.
  • Submission Requirements: Must have an active drivers license with submission; submit RTO requests during application.
