Job Summary
Occupational Therapist Home Health Missoula, MT. The Occupational Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education. This role involves coordinating care with multidisciplinary teams, providing direct patient care based on established care plans, and ensuring compliance with regulatory standards.
Job Details
- Certifications, Education, and Experience: BS degree or higher from an accredited Occupational Therapy program, current license to practice in the State of Montana, valid drivers license, and appropriate auto insurance. Two years of previous OT experience preferred.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
- Evaluates and establishes care plans to meet patient needs in the home environment.
- Coordinates care with other disciplines and seeks orders for additional services as necessary.
- Provides direct patient care based on the established plan and physician orders.
- Applies knowledge of occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations, and treatment techniques.
- Modifies treatment as appropriate, including frequency and duration of care.
- Masters scoring of the CMS required OASIS and collaborates with the Quality team to make needed adjustments.
- Acquires and maintains a basic understanding of insurance requirements in home health.
- Participates in multidisciplinary conferences for developing relevant care plans.
- Communicates appropriately with physicians and multidisciplinary team members.
- Documents all skilled services, adverse events, and relevant communications in the medical record.
- Participates in Quality Assurance Performance Improvement initiatives.
- Manages and meets productivity standards in line with the organizations standards.
- Maintains accurate time logs and mileage, following agency policies.
- Communicates with the scheduler daily to optimize care continuity and travel routes.
- Oversees services provided by the Occupational Therapy Assistant and performs supervisory visits within required time frames.
- Supervises CNAs when other disciplines are no longer involved in the care plan.
- Takes part in relevant care conferences and plans admissions, OASIS time points, and discharges throughout the episode of care.
- Maintains occupational therapy licensure requirements, including continuing education.
- Practices according to ethical standards as defined by the licensure board and organizational policies.
- Participates in educational opportunities as scheduled.
- Performs other duties as assigned by the supervisor.
